Where do I put the sitemap?

Upload it to your site, commonly at https://example.com/sitemap.xml, then reference it in robots.txt and submit it in Google Search Console.

Are changefreq and priority required?

No. They are optional hints and search engines largely treat them as advisory. A clean list of valid <loc> URLs is the most important part.
